Title: South team home care Chaplain

Job Summary:

Duties: A. Provide spiritual and emotional support to Hospice patients and their families and loved ones. B. Represent the Pastoral Department in liaison between the Hospice of Saint John and external clerical persons and institutions. C. Participate in meetings with IDT, UR and QI teams; share confidential information with them as necessary, on a need-to-know basis. D. Fulfill Department of Health and Medicare regulations by interviewing patients and families and writing spiritual and emotional assessments on a timely basis. Document Hospice adjustment, safety risks and patient concerns for IDT Care Plan. E. Provide educational services for other Hospice staff, in the form of in service instruction and ad-hoc assistance to other disciplines, to improve overall Hospice patient care. F. Administer the use of the Hospice repository, under direction and guidelines provided by the Director of Pastoral Services. PST Policy 1.2 (Cont.) G. Call indicated parties at times of significant patient events, such as patient watch, as indicated by Nursing and other disciplines. Document such events and calls in the patient’s progress notes. Minister during watch period and at time of death to patient, family and Hospice staff, as appropriate, to the best of Chaplain’s abilities. H. Handle the administrative duties contingent on death, so as to provide a strengthening and supportive atmosphere for the bereaved. This may include, but is not necessarily limited to, informing absent relatives and friends, comforting bereaved with solace and prayer, calling the mortuary once the family has requested this, and patient has been medically released. Document death in final entry of patient’s progress notes, using format prescribed by Director of Pastoral Department. I. Refer family and patients to Hospice bereavement services, where appropriate. Refer to other agencies or local parish or church community of choice, if they can provide support the Hospice cannot. J. Take necessary steps to ensure that emotional and spiritual support from internal or external sources does not obtrude on patient’s and/or family’s belief system, and that such support does not proselytize or attempt to introduce or impose any doctrine or belief system which is not germane or welcome to the patient and/or family. This applies particularly to attempts by any person to convert or change any aspect of the patient’s belief system as a prerequisite of divine favor or salvation. Such behavior is contrary to Hospice’s philosophy, is considered a breach of trust between patient and chaplain, and is grounds for immediate termination. K. Conduct or be present at daily morning prayers whenever possible. Ensure that these are conducted in a manner consonant with the philosophy and vision of the Order of Saint John, using the formats provided. L. Where and when needed, assist with tours for prospective Hospice patients and/or their families. Special Note: It is permissible, if a patient’s family so requests, for the Chaplain to arrange and conduct funeral and memorial services, especially when a personal minister or particular church is not involved. This must be arranged to take place at a time when the Chaplain is not on duty with the Hospice or if that Chaplain makes adequate arrangements with other Pastoral personnel to cover their duties. The Chaplain must also inform the Director of Pastoral Services of these arrangements. No Hospice insignia will be worn for such occasions. Any gratuity given in return for such service may be kept by the Chaplain. Since this is an extracurricular activity undertaken by the Chaplain on a voluntary basis, no mileage reimbursement or other remuneration will be payable by Hospice. M. Serves as liaison with the church, synagogue, or parish with which the patient/family is associated. PST Policy 1.2 (Cont.) N. Aids family at patient’s death with mortuary notification O. Assists in making funeral plans, if the family desires assistance. P. Follows pastoral care of Home Care patients transferred into inpatient hospice setting for respite care. 1. When a patient becomes a resident within the facility, the home care chaplain will assist in the transition of care to the inpatient chaplains. Home care may, if they wish, continue to be involved in the pastoral care of the patient. 2. When a patient in the facility become a home care patient, the in patient Chaplain will contact the home care Chaplain and communicate pertinent information. Q. Medical Records and Documentation responsibilities 1. Patient charts: Documents all correspondence pertaining to pastoral phone calls, visits and mortuary notification and other pertinent information. R. Chaplains duties and work times will be established by the Director of Pastoral Services. All Chaplains must be proficient in the discharge of duties for both In-patient and Home Care responsibilities and On Call. The work hours may include regular work times and on call coverage during weeknights and weekends.

Requirements:

QUALIFICATIONS AND PREREQUISITES

Desired: Degree-level studies in theology or religious studies. Extensive and long-standing involvement in church-based activities and ministries.

Required: Clinical Pastoral Education or Stephen Ministry training, or an equivalent academic certification acceptable to the Director of Pastoral Department.

Experience One year minimum in a pastoral service role, preferably in a situation involving patients with terminal conditions.
